Dance Class Participation Waiver & Release of Liability

By enrolling in and participating in dance classes, workshops, camps, events, or other activities offered by Upperroom Dance Studio, I acknowledge and agree to the following:

1. Assumption of Risk

I understand that dance and physical activities involve inherent risks, including but not limited to muscle strains, sprains, fractures, falls, and other injuries. I voluntarily choose to participate and assume all risks associated with these activities.

2. Health Declaration

I confirm that I am physically able to participate in dance classes and have no medical condition that would make participation unsafe. If I have any existing injury or medical condition, I understand that it is my responsibility to consult a healthcare professional before participating and to inform the instructor prior to class.

3. Release of Liability

To the fullest extent permitted by law, I release and hold harmless Upperroom Dance Studio, its owners, instructors, employees, contractors, volunteers, and representatives from any claims, demands, liabilities, damages, or expenses arising from any injury, illness, accident, loss, or damage sustained while participating in studio activities, except where caused by gross negligence or intentional misconduct.

4. Personal Belongings

Upperroom Dance Studio is not responsible for any lost, stolen, or damaged personal belongings brought onto the premises.

5. Conduct & Safety

I agree to:

• Follow all instructions given by instructors and staff.

• Wear appropriate dance attire and footwear (if required).

• Respect fellow students, instructors, and studio property.

• Refrain from disruptive, unsafe, or inappropriate behaviour.

• Immediately inform an instructor if I experience pain, dizziness, or any medical concern during class.

Upperroom Dance Studio reserves the right to remove any participant whose behaviour compromises the safety, comfort, or experience of others.

6. Photography & Video

I understand that photos and videos may occasionally be taken during classes, workshops, performances, or studio events for promotional, educational, or social media purposes.

By signing this waiver, I grant Upperroom Dance Studio permission to use my image or likeness in these materials without compensation. If I do not wish to appear in photos or videos, I will notify the studio in writing before participating.

7. Class Policies

I understand that class schedules, instructors, and programmes may change when necessary. Missed classes, cancellations, refunds, and transfers are subject to the studio's current policies.

8. Minors

If the participant is under 18 years of age, this waiver must be signed by a parent or legal guardian, who agrees to all terms on behalf of the participant.

Acknowledgement

By signing below, I confirm that:

• I have read and understood this Dance Class Participation Waiver & Release of Liability.

• I understand the risks involved in participating in dance activities.

• I voluntarily agree to all of the terms stated above.

• I understand that this waiver applies to all classes, workshops, camps, rehearsals, performances, and events conducted by Upperroom Dance Studio unless revoked in writing or replaced by a new agreement.
